To enter: Visit us online at www.cvs.com/naturemade and complete the registration page, or hand print your name, address, phone number, and email address on a 3 x 5 card and mail to: Department 33361, Nature Made Sweepstakes, PO Box 52900, Phoenix, AZ 85072. Limited to one entry per person/email address. Sponsor and its agents not responsible for lost, late, incomplete, mutilated, misdirected or illegible entries. All mail-in entries must be postmarked by 7/10/02.
Sweepstakes begins on 4/2/02 and ends on 7/1/02. Open to residents of the 50 United States and District of Columbia who are 18 or older. Employees of PharmaviteŽ LLC., Continental Promotion Group, Inc, their affiliates, subsidiaries, advertising and the immediate family members of each are not eligible to enter. Void where prohibited.
One (1) Grand Prize winner will receive a two night spa getaway trip for two to the Miraval Resort in Catalina, AZ. Trip includes coach airfare from major airport nearest winner's home, airport transfer from Tucson airport to Spa, two nights deluxe accommodations for two, three gourmet meals per day and unlimited non-alcoholic beverages, one spa treatment, round of golf, or one-on-one consultation with a golf tennis or fitness pro per person per day, and access to all group programs and activities. Approximate retail value of Grand Prize is $4,275.00. Six (6) First Prize winners will receive $100 worth of Nature Made vitamins from their custom Nature Made Wellness Advisor Vitamins Regimen. Approximate retail value of each prize is $100.00. Total approximate retail value of all prizes is $4,845.00 (four thousand eight hundred forty five dollars).
Winners will be chosen in a random drawing conducted on or about 7/15/02 by an independent judging organization whose decisions shall be final on all sweepstakes matters. Winners will be notified by mail. Odds of winning depend on number of entries received. Winners may be required to sign affidavits of compliance and eligibility and a liability/publicity release within 14 days of notification or prize may be forfeited and awarded to an alternate winner. Unclaimed prizes will not be awarded. Sponsor reserves the right to substitute prizes of equal or greater value.
Winners are responsible for any local, state, and federal taxes. Winners consent to sponsor's use of their name, likeness or voice for any advertising or promotional purpose without further consent or compensation, except where prohibited. Winners agree to release sponsor, its employees and agents, from any and all liabilities resulting from entry, possession or use of any prize. Sponsor and its agents are not responsible for any printing or technical error which may occur in processing, printing, or distribution of the sweepstakes materials.
